Pneumonia vaccine - $ 20.00

Forms will be provided
by the Ocean County
Health Department at
the Clinic and assistance
will be provided.

(The target population for the Pneumonia vaccination
Is persons aged 65 & older. Anyone that is
not in this age group must have a note
from their physician prior to the vaccination.
Anyone age 9 & older is eligible for the
flu vaccination)
